Rend+Triple Options

If a character with Rend and Double Attack gains Extra Attack, leaving him with three attacks, how does Rend work? My thinking was that it would apply to both the second and third attacks (as Attacks 2+3 are a subset of two attacks), but I'm probably wrong.

Nope, it only applies if the first and second main (not Twin) attacks hit.
