Can someone please confirm for me if the following is true or false:
1) The ithorian commander's plus 4 for attack does not stack with Kota's plus 3. You just apply the higher plus 4 and ignore the plus 3 from Kota.
2) Similarly, jabba's plus 2 attack for bh does not combine with Kota's plus 3 for attack.
3) Basically, no commander effects can ever stack for the same purpose. You can only ever have a bonus for attack once and a bonus for defense once.
4) Princess Leia gives one ally within 6 one immediate shot. Let's say it's to a ERC and Admiral Ackbar is within 6. Does the ERC get Leia's plus 4 for attack and Admiral Ackbar's plus 4 for attack? I assume it does not stack as well.

Right, raw stat bumps never stack from Commander Effects, but you can get stat bumps from several different sources to work, so Cunning, Momentum, and a CE might all work on one character but not multiple commander effects just giving +4/+4
This can probably explain it better than I can. From the FAQ
Stacking Q: If have a Battle Droid within 6 squares of General Grievous, Supreme Commander, and I have a Battle Droid Officer in the same squad, would the Battle Droid get +4 or +8 to attack? Should these bonuses stack? A: The Battle Droid would get +8 to attack in addition to getting Double Attack from General Grievous, Supreme Commander. These bonuses do indeed stack. The bonus provided by General Grievous, Supreme Commander is from a commander effect, but Fire Control is not. (If both bonuses were from commander effects, they wouldn't stack; you'd just get the better of the two bonuses.)

Always glad to help! I hope that made sense, stacking is something that people often have a hard time getting their heads around what exaclty works with what.

Also, note that if the CE grants an ability that boosts attack, that would stack with a raw boost. For example, if Xizor and Mith-Thrawn (not Grand Admiral Thrawn) are in a squad together, a Bith Black Sun Vigo could get both Opportunist and the +4/+10 from Xizor. That would be +8/+20 versus an activated enemy.

That would be because Thrawn's opportunist is only sometimes and Xisor's is always true as long as within 6, right? The distinction of one being sometimes and one being always is what allows them to be together, right?

so, it is the fact that one is an SA (opportunist), and one is a CE that gives a flat bonus.
You can use CEs to give as many named abilities as you can.
For instance if you used the Weequay Leader, Grand Admiral Rulf Yage (GARY), Lord Vader, and Grand Admiral Thrawn a stormtrooper would get Mighty Swing, Cunning, Advantageous attck, AND the +3/+3
for a grand total of (+7/+30) of you were attacking an adjacent enemy who hadn't activated and declared that you weren't moving so you could benefit from mighty swing.
