Can Thrawn swap an ugernaut for Vader? Or is an ugernaut considered small and Vader a Medium? If that is so, how can you tell? Is it based solely on the bases of the characters being a tiny bit smaller?

It's due to the base size - the base size is listed in the Bloomilk database too: eg http://www.bloomilk.com/Character/530/uggernaught is Huge.

Small and Medium both fit on one square on a map, but there's an obvious size difference.

You cannot make that swap. As you say, the difference between small and medium is just the size of the base. When in doubt, bloomilk lists the base size on each character's page. I assume you meant Ugnaught Demolitionist (the most commonly used ugnaught), who has a small base. If you meant Uggernaught, that has a huge base.

Large base models take up 4 squares and are all considered the same size, even though physically some are larger than others. Same thing for huges, which take up 9 squares each.
