How does Opportunist work?

If you have a character that has double/twin, and he's shooting someone who has activated, would each attack have a +4 attack at +10dmg? Or would it just be the first attack? Because it doesn't say anywhere that "all attacks" get +4 attack and +10dmg. Just curious.

It's any attack against an enemy who has been activated. It's exactly the same situation as with Cunning, Jedi Hunter, Bounty Hunter, etc.

A character with Opprotunist gets +4 attack and +10 damage on ALL attacks against character that have activated. So let's say you give Cad Bane Opprotunist and he shoots at GMLS who has already activated. For his first attack and second attack (along with the Twin Attacks that follow), you'll get Opprotunist for a total of a 20 attack (+6 for Bounty Hunter and +4 for Opprotunist).

You may be getting some CEs that allow a character to attack at the end of said commander's turn. For example:

Princess Leia activates, shoots, then moves. At the end of her turn, she lets Luke Skywalker's Snowspeeder shoot. Luke's SS has Twin, but only the first shot gets the +4 attack and +10 damage. So Luke would shoot with a 16 attack for 30 damage followed by a Twin for his normal 12 attack for 20 damage.
