If someone has melee reach 2 or 3, do they get an AOO against anyone moving through that range or into contact with them?

Yes, as they are considered adjacent when they are within the stated number of squares.

A character with Melee Reach X is only considered adjacent for the stated range on THEIR OWN turn while attacking. For it to work with an AoO, you would need the enemy to move during an attack, which won't be happenning 99.9999999% of games.
