Ahsoka gets jealous of Padme and blames Anakin for not training her and focusing more on Padme,and then decides to elminiate Padme. She does, and Anakin kills her in return. The dirty love triangle.

With 20 HP remaining, she tries to move away from an adjacent battle droid who crits on the AoO and she fails the Lightsaber Deflect save. :)
In the series, I can see Cad Bane killing her in front of Anakin. He gets all emotional and goes berserk and kills Bane in rage (maybe cuts off his feet and ankle-boot jets as he flies away, then lightsaber through the chest of defenseless Bane) and takes a step to the dark side. Both other main cartoon characters are removed before RotS starts.

But it will be something Lame like Rex and Cad Killing themselves in some sort of Epic fight (come to think of it, I like that idea too!)

Rex eventually becomes the leader of the 501st under Vader under a different name than Rex so Rex isn't going to die. Cad at this point is 50/50. I for one don't believe and haven't believed that Ahsoka will die. It doesn't make sense storyline, character, or canonically. She will live, possibly as one of the Jedi who escaped the purge. She will not die by Anakin's hand, Rex's hand, if anyone is going to kill her I imagine Dooku or Durge will do it.
