Does disruptive affect Grand Moff Tarkin's CE if Tarkin himself isn't in the disruptive range?

No. Tarkin's CE affects the player, and the player is never on the map (hopefully). ;)

Related question - what if you have San or Ozzel, and you move them into disruptive. Do you get a second activation that phase?

No, because they are triggered at the beginning of the phase. So if their CE is active at the start of the phase, it remains in effect, even if they move within Disruptive range.
