If whorm gives him twin, how does that work with rend 20? Can he get the +20 damage twice?

The first and the Double Claw Attack are what trigger Rend, not the Twin Attacks. You can only get Rend once, no matter how many attacks you make. The boost depends on the first and second main attack.

1) So, as long as the 1st and 3rd attack hit rend 20 kicks in, right?
