Actually, I find that Bloomilk is the best spot; just click the "Customs" tab near the top of this page.
As for setting up the graphics, I'm not sure. I know that some people around here have found a way to produce some pretty sweet-looking cards, though.

Here is my best crack at it using paint.net I had to shrink some words down and I see I missed an entire line in FL 2 but I think the card is readable. I found blank card templates on the old WotC site then filled in text and stole the bane pic from the bloomilk image

For quick, basic formatting and layout purposes, I use PowerPoint.
For permanent designs... I don't know. I haven't made any yet. When I do make some, I'll probably use GIMP (which is a free, open-source program)
I could provide more specific information about the card background design and the fonts if you would like.
