Do multiple instances of extra attack, both gained from CEs, stack? Or do they only stack once with double/triple?

From the glossary text:
"This extra attack stacks with those granted by other special abilities and commander effects, and it also stacks with Extra Attacks granted by different special abilities (so the same ally could receive multiple extra attacks in the same turn)."
I would assume so, but I could be wrong.

Yes, Extra Attack stacks with itself (and with Double, Triple, what have you).

Yes, they stack. I believe "Extra Attack" is one of the few (if not the only one) things that can stack from multiple CE's.
