would the ithorian commander, ackbar, and kota's ce's stack w/ each other?

No, because they are all generic bonuses to the attack rating, rather than granting a named special ability that also boosts attack (i.e. - Cunning, Opportunist, etc etc).

is it only for commander effects?

for example, does the battle droid officers "fire control" (+4 atk) SA combined with GGDAC's (+4 atk) CE violate the stacking rule since it's not a commander effect?

Confused confused? it's fairly easy.
CEs stack with SA. CEs stack if they have different names.

Is Fire Control a CE that grants a generic bonus?

Fire control is a special Ability. It stacks with any CE.Mellow

Location: Desintegrating some Djem So Sucka!
For example, jabba the hutt from RS grants a +2 to all bounty hunter followers (except IG-88 and 4-Lom, they are droids).(generic modifier)
Master Kota grants a +3 to fringe followers within 6 squares.(generic modifier)
And The Hated GOWK grants followers a +4 (generic)to att and Def if an ally is within 6 squares.
those abilities would not stack because they are generic.
generic in the sense that they only grant +2, +3 and +4.
Careful shot grants a +4 to your attack, and that is a named ability, not a "generic".
so you could stack GOWKs CE with a Merc commanders Dead eye CE, because one is named, and the other is just "generic".

Cunning Attack (+4 attack, +10 damage)
Advantageous Attack (+10 damage)
both against unactivated enemies

Opportunist + Deceptive
Deadeye, Mighty Swing, Rend, Poison etc

and then any of those on top of generics - Kota, Yoda, GA Thrawn, Ithoran Commander, Xixor etc (depending of course on there being characters that could take advantage of it)
